Microbiome & fertility - Why intimate balance matters more than we think

Infertility is far more common than many realize. According to the World Health Organization (WHO), one in four couples in developing countries is affected. And yet, despite how widespread it is, we rarely talk about it.

Fertility struggles are often surrounded by silence, shame, or discomfort. Many women carry this burden quietly — sometimes without even sharing it with those closest to them.

A changing landscape in fertility

Since the birth of the first IVF baby in 1978, fertility treatments have advanced tremendously. Today, IVF offers hope to many — but success is not always guaranteed.

Around 10% of women undergoing IVF experience recurrent implantation failure (RIF). This means that pregnancy does not occur despite the transfer of multiple high-quality embryos.

So what could be missing?

The emerging role of the microbiome

Recent research points to a fascinating and important factor:
the vaginal and endometrial microbiome.

Studies show that women experiencing fertility challenges, including RIF, often have an imbalance in their vaginal microbiome — with lower levels of protective Lactobacillus.

As Swedish gynecologist and obstetrician Dr. Rebecka Kaplan explains:

“An unfavorable microbiome in the vagina and uterus may be linked to reduced fertility, increased risk of miscarriage, and a higher likelihood of premature birth. The bacterial balance plays a crucial role in sperm survival, embryo implantation, and in protecting both the pregnancy and the baby from infection.”

Why balance matter

There is still much to learn. But one thing is already clear:
